MNT is a healthcare service provided by Registered Dietitians (who are also licensed in the state of GA). The goal of MNT is to prevent, delay, or manage nutrition-related medical concerns or conditions using an evidence-based, individualized process. 

Yes, ages 4 years and up.

Yes. A referral from a medical provider (MD, NP, or PA) is required for ALL insurance-based patients and can be faxed to the office at 478-200-9588. This will help us properly check insurance benefits coverage prior to the appointment and communicate any co-pays or co-insurance costs. 

A nutrition referral form can be emailed upon request for you to take with you to your next doctor's appointment.

Yes, many commercial insurance plans cover MNT for a variety of diagnoses including obesity, overweight, diabetes, prediabetes, hypertension, and cardiovascular diseases. You may can call the number on your insurance card to ask about MNT benefits on your specific plan. 

For Medicare patients with a diagnosis of diabetes and/or chronic kidney disease, MNT is covered at 100%. Nutrition counseling for all other diagnoses will be provided for an out-of-pocket fee.

Yes. If you are unable to travel to the primary office location in Forsyth, GA for an in-person visit, virtual video visits are an option (and may also be covered by your insurance). For pediatric appointments, please note that both the child and caregiver must be present on the video call.
